You can now access your Madison Radiology imaging reports through our new online patient portal. Our patient portal allows you to easily access your imaging results anytime, anywhere.

With our patient portal, you can:

  • View and download your reports from the last three years.
  • Share results with your care team.
  • View and manage your upcoming and previous appointments within the past 30 days.
  • Complete required forms in advance of your appointments.
  • Upload and update your insurance card information.
  • View, add, and remove authorized users who are able to access your Patient Portal.

How do I access the patient portal?

To be able to access the patient portal, you MUST have an email on file with us already. If you do not have an email on file with us, please fill out this form: https://guilfordradiology.com/request-access

  1. Visit the link above.
  2. Click on "create account".
  3. Fill out the requested personal information, create a username and password, then click register.
  4. Check your email for a confirmation message from [email protected] and click on "activate account".

After activating your account, you can see your dashboard after logging in.

Our History - Madison Radiology

Bernard Jay, M.D. founded Madison Radiology in 1979.   Dr. Jay started the radiology practice by converting a single floor condo space into a mammography center.  He worked many years as the sole practitioner at Madison Radiology and over the years he expanded the practice to include a women's imaging center adding ultrasound, and bone densitometry.  Seeing the need for more affordable outpatient imaging, he added Xray, CT, and fluoroscopy.  Dr Jay was proud of the quality work he and his dedicated staff did to service their patients.

In 2022, Dr. Jay approached the partners at Radiologic Associates of Middletown (RAM Radiology) about partnering with Madison Radiology.    RAM Radiology saw the opportunity to continue to deliver high level care on the CT shoreline and knew the Madison Radiology would be a great addition to their existing Guilford Radiology facility in Guilford CT.  They saw a need and opportunity to continue to provide high quality medical imaging in a professional outpatient setting on the Connecticut shoreline.  

RAM Radiology acquired Madison Radiology in February 2023.  RAM is updating the facility and upgrading equipment to continue the work of Dr. Jay and providing the best imaging on the CT Shoreline.
